School Forms

Westside School is committed to keeping each student's confidential health and emergency contact information up to date and accessible to your child's teacher. In order to do this, we must have updated school forms from each family, for every student, every academic year. There are two different types of forms. The first are forms which are sent to families via email as attachments. These forms should be returned to Westside School (preferably via email or fax, or by mail or by dropping them off at the school.) The second are online forms, which come directly from our student database. These forms must be completed and submitted once you are logged into your account, and cannot be printed.

The following forms will be requested before each new academic year, and we respectfully request you strictly

adhere to posted deadlines.

Lower School

Medication Administration Authorization form (this form must be signed by the parent and the child's physician for both prescription drugs and OTC medication)

FARE Food Allergy form (this form must be completed for children with food allergies, and signed by the parent)

Certificate of Immunization Status form (to be considered COMPLETE this form must be signed by the parent and the child's physician)

Certificate of Exemption (to be considered COMPLETE, this form must be signed by the parent and the child's physician OR church to which you are a member)

Medical Information and Permission for Emergency Treatment Form (this online form is required for all students before the first day of school)

Emergency Contact/Release Authorization Form (this online form is required for all students before the first day of school)

Middle School

Participation History and Physical Examination (all students participating in Middle School Athletics must have a physical, and this form must be signed by their physician)
